Online Medical History Questionnaire
When you are ready to take the next serious step forward, click the link below, titled “Online Patient History Form”. A .pdf file will open and you may then begin providing the information which will allow us to set up an initial appointment, by phone or in my office. It is at that time we will be able to answer your questions about (1) whether TMS might be a good fit for you and you for Southern Colorado TMS Center, (2) the estimated cost of TMS treatment for you and (3) the possibility of your health insurance covering all or part of the cost.
When the form is completed, press the RED BUTTON at the bottom of the last page. A popup will explain how you may best return the Patient History Form to us. However, the simplest method is simply to “SAVE” the .pdf file onto your desktop, open your regular email program and compose a new email. Address this to

and then “attach” the saved Patient History Form to your email. At that point, we are ready to meet on the phone or in my office, answer questions, and then to decide on your next step.
